Tiburon Hills Care Center has been a great partner in providing continued care for patients who still require skilled nursing services after discharge. Their team is responsive, professional, and focused on making the transition as smooth and safe as possible. When a patient is not yet ready to return home and needs additional skilled nursing care, Tiburon Hills works closely with the discharge team to review the patient’s needs, coordinate care, and help ensure the appropriate support is in place.
